What Causes Shoulder Pain?
The shoulder is a complex region, and pain can have several different causes.
Common shoulder problems include conditions affecting the:
- Rotator cuff
- Subacromial bursa
- Shoulder joint
- Acromioclavicular (AC) joint
- Biceps tendon
- Surrounding muscles and tendons
This is one reason why simply requesting a shoulder steroid injection without first establishing the likely diagnosis may not be the best approach.
The location of your pain does not necessarily identify which structure is responsible.
What Is a Steroid Injection?
A corticosteroid—or steroid— injection is a medication that may be used to reduce inflammation and pain in selected musculoskeletal conditions.
For appropriately selected patients, reducing pain may also make it easier to move the shoulder and participate in rehabilitation.
However, steroid injections are not suitable for every type of shoulder pain. The decision to inject should therefore follow an appropriate clinical assessment.
When Can a Steroid Injection Help Shoulder Pain?
Whether a steroid injection is appropriate depends on the underlying diagnosis, severity and duration of your symptoms, previous treatment, medical history and individual circumstances.
For certain shoulder conditions, an injection may form part of a wider treatment strategy.
For others, rehabilitation or further investigation may be more appropriate.
This is why my approach is:
ASSESS → SCAN → DIAGNOSE → TREAT → REHABILITATE
rather than:
PAIN → INJECTION
Why Diagnosis Matters Before a Shoulder Injection
Two patients can both describe having “shoulder pain” while having very different underlying problems.
Before considering an injection, I take a detailed history and perform a comprehensive physical examination.
Where clinically appropriate, I can then perform a diagnostic musculoskeletal ultrasound scan of the shoulder.
The scan findings are interpreted alongside your symptoms and physical examination rather than being considered in isolation.
This helps determine whether an injection is appropriate and, importantly, where treatment should be targeted.
What Can a Shoulder Ultrasound Scan Show?
Diagnostic musculoskeletal ultrasound provides real-time imaging of many of the soft-tissue structures around the shoulder.
Depending on the clinical question, ultrasound may help assess structures including the:
- Rotator cuff tendons
- Long head of biceps tendon
- Subacromial-subdeltoid bursa
- AC joint
- Surrounding soft tissues
Ultrasound can also allow certain structures to be assessed dynamically while the shoulder moves. However, ultrasound is not the correct investigation for every shoulder condition.
If your assessment suggests that another investigation such as an MRI or X-ray is more appropriate, this can be discussed and arranged when clinically indicated.
What Is an Ultrasound-Guided Shoulder Injection?
An ultrasound-guided shoulder injection uses real-time ultrasound imaging while the procedure is being performed.
This allows me to visualise the relevant anatomy and guide the needle towards the intended treatment area.
Rather than viewing the scan and injection as two unrelated procedures, they can form part of the same clinical pathway:
Assessment → Ultrasound → Diagnosis → Injection when appropriate → Rehabilitation
Can I Have My Shoulder Scan and Injection at the Same Appointment?
Yes—when clinically appropriate.

Do Steroid Injections Cure Shoulder Pain?
A steroid injection should not automatically be viewed as a cure.
Pain reduction can be valuable, but it is important to consider the wider reasons why the shoulder has become painful and what is required to restore function.
For some patients, reducing pain may provide an opportunity to progress rehabilitation more effectively.
This is where combining injection therapy with physiotherapy can be particularly useful.
The goal is not simply to make the shoulder feel better temporarily.
It is to help you return to the activities that matter to you.
Should I Have Physiotherapy or a Shoulder Injection?
It isn’t necessarily a choice between the two.
Depending on your diagnosis, physiotherapy may be the primary treatment.
In other circumstances, an appropriately selected injection may help manage symptoms sufficiently to allow rehabilitation to progress.

What If a Steroid Injection Isn’t Appropriate?
An important part of a specialist musculoskeletal assessment is recognising when not to inject.

- Activity modification
- Further diagnostic imaging
- MRI
- X-ray
- Medical or specialist referral when indicated
