If you are considering a steroid injection for joint, tendon or musculoskeletal pain, you may have come across two different approaches: ultrasound-guided injections and blind or landmark-guided injections.
But what is the difference, and does ultrasound guidance matter?

One of the major advantages of ultrasound is that it allows me to assess the area and, where an injection is clinically appropriate, visualise the target and needle in real time.
In this article, I explain the difference between ultrasound-guided and blind injections and why accurate diagnosis should come before deciding to inject.
What Is an Ultrasound-Guided Injection?
An ultrasound-guided injection uses real-time diagnostic ultrasound imaging to help identify the anatomical structure being treated and guide the needle towards the intended target.
Musculoskeletal ultrasound can provide detailed real-time images of structures including:
- Tendons
- Muscles
- Bursae
- Joints
- Ligaments
- Fluid collections
- Other superficial soft tissues
During an ultrasound-guided steroid injection or other musculoskeletal injection, I can observe the needle on the ultrasound screen as it approaches the intended target.
This differs from a blind injection because the procedure does not rely solely on anatomical landmarks to estimate the target’s location.
What Is a Blind or Landmark-Guided Injection?
A blind injection, more appropriately described as a landmark-guided injection, is performed without ultrasound imaging.
The clinician uses their knowledge of anatomy, clinical examination and external anatomical landmarks to determine where the needle should be placed.
Landmark-guided injections have been performed successfully for many years and can remain appropriate for certain conditions and anatomical locations.
The fundamental difference is that during a landmark-guided procedure, the clinician cannot directly visualise the needle and target using ultrasound.
Ultrasound-Guided vs Blind Injections: What’s the Difference?
The easiest way to understand the difference is:
Blind/landmark-guided injection:
The clinician estimates the location of the target using examination and anatomical landmarks.
Ultrasound-guided injection:
The clinician uses real-time ultrasound imaging to visualise the relevant anatomy and guide the needle towards the intended target.
This can be particularly valuable when treating smaller or deeper structures or when the surrounding anatomy needs to be carefully considered.
Are Ultrasound-Guided Injections More Accurate?
One of the main reasons for using ultrasound guidance for musculoskeletal injections is needle-placement accuracy.
Research involving a range of musculoskeletal injection procedures has demonstrated improved accuracy of needle placement with ultrasound guidance compared with landmark-guided techniques.
If medication needs to be delivered to a specific joint, bursa, tendon sheath or other anatomical structure, being able to visualise that structure and the needle can help the clinician accurately target the intended location.
Greater injection accuracy does not guarantee a better clinical outcome for every patient or every condition.
An accurate injection will not compensate for an incorrect diagnosis or an inappropriate treatment choice.
This is why I believe the assessment before an injection is just as important as the injection itself.
Diagnosis Before Injection
One of the biggest advantages of attending a clinic offering both diagnostic musculoskeletal ultrasound and ultrasound-guided injections is the opportunity to investigate what may actually be causing the symptoms.
Pain does not always identify the damaged or irritated structure.
For example, shoulder pain can potentially arise from several different structures and conditions. Simply injecting the painful area without establishing an appropriate working diagnosis may therefore not be the best approach.
My approach is:
EXAMINE → SCAN → DIAGNOSE → DISCUSS → TREAT
Where clinically appropriate, I can combine a detailed physical examination with a diagnostic ultrasound scan during the same appointment.
We can then discuss the findings and determine whether an injection is appropriate.
Sometimes the most important result of an ultrasound assessment is deciding not to inject.
Seeing the Needle During an Ultrasound-Guided Injection
During an ultrasound-guided procedure, the needle can be visualised on the ultrasound monitor as it travels towards the intended target.
This allows the procedure to be adapted to the patient’s individual anatomy rather than relying entirely on predetermined anatomical landmarks.
Ultrasound can also help the clinician consider surrounding anatomical structures when planning the needle pathway.

What structure are we treating?
Why are we considering an injection?
What happens after the injection?
I believe patients should understand why a procedure is being recommended rather than simply receiving an injection into the area that hurts.
Are Ultrasound-Guided Steroid Injections Safer?
No injection procedure is completely risk-free.
The potential risks depend on the type of injection, medication being used, anatomical location, medical history and individual patient circumstances.
Ultrasound guidance provides real-time visualisation of the needle and surrounding anatomy, which can assist with planning and performing the procedure.
However, ultrasound cannot eliminate every potential complication.
Appropriate patient selection, clinical assessment, informed consent, sterile technique and appropriate aftercare therefore remain essential.
Do All Steroid Injections Need Ultrasound Guidance?
No.
It would be inaccurate to claim that every steroid or musculoskeletal injection must be performed using ultrasound guidance.
Experienced clinicians can appropriately perform certain injections using anatomical landmarks.
There are also circumstances where an injection may not be the correct treatment at all.
Rather than asking only:
“Can you give me a steroid injection?”
A better starting point may be:
“What is causing my pain, and is an injection appropriate for my condition?”
That distinction is central to how I assess patients at LG Physiotherapy, Ultrasound & Injection Clinic.
What Conditions Can Be Assessed for Ultrasound-Guided Injections?
Following an appropriate clinical assessment, ultrasound-guided injections may be considered for selected musculoskeletal conditions involving areas such as the:
Shoulder | Elbow | Wrist & Hand | Hip | Knee | Ankle & Foot
Depending on the diagnosis and individual circumstances, an injection may potentially target a joint, bursa, tendon sheath or another appropriate anatomical structure.
Not every painful tendon, joint or soft-tissue structure should be injected.
The decision should be based on the diagnosis, medical history, examination findings, previous treatment and an informed discussion of the potential benefits and risks.
Ultrasound-Guided Shoulder Injections
Shoulder pain is one of the common reasons patients seek musculoskeletal assessment.
However, “shoulder pain” is not a diagnosis.
Different structures can contribute to symptoms, which is why clinical assessment and, where appropriate, diagnostic shoulder ultrasound can be useful before deciding on treatment.
If an injection is appropriate, ultrasound can then be used to guide the needle towards the selected anatomical target.
Ultrasound-Guided Joint Injections
For selected joint problems, an ultrasound-guided joint injection may form part of the treatment plan.
Ultrasound allows the joint and surrounding anatomy to be assessed and can provide real-time guidance during needle placement.
The suitability of an injection depends on the particular joint problem, symptoms, examination findings and the patient’s wider medical history.
Ultrasound-Guided Injections Shouldn’t Replace Rehabilitation
An injection should not automatically be considered a standalone cure for a musculoskeletal problem.
For many patients, an injection is only one component of treatment.
Depending on the diagnosis, rehabilitation may include:
- Progressive strengthening
- Physiotherapy
- Load management
- Mobility exercises
- Modification of aggravating activities
- Gradual return to exercise or sport
An injection may help create an opportunity to progress rehabilitation when pain has been limiting recovery.
But the rehabilitation still needs to happen.
